All students must have the required health forms submitted before August 16th, 2024 to be reviewed before the start of the school year.
Families need to ensure students are up to date with all of the required vaccines and should consult with their doctors. If students are not fully immunized, and do not provide proof of immunization within the first 2 weeks of the school year they will be excluded from school until they receive the required vaccines.
**All forms must be returned by hard copy via mail or directly delivered to the school. This is in compliance with standards of safeguarding for student medical information. Submission Details: Please send the completed forms to:

Please join the summer math for the course you will be taking in the fall. For incoming 9th grade students, if you have taken Algebra 1 already, please complete the Algebra 2 summer work. If not, please complete the Algebra 1 assignments. Incoming 10th grade students will take Geometry.
